The following client forms utilize EchoSign, a trusted and highly secure provider of electronic signing for online forms. These documents are encrypted and transmitted automatically to InfoRelay Online Systems, Inc. upon signing. United States law permits electronic signatures through the Electronic Signatures in Global & National Commerce Act.

- CLIENT CONTACT FORM – For relaying authorized billing, technical and management contacts.

- NAME CHANGE REQUEST – Use this form to change any names associated with a client account.
